Union Square Dental Practice (“Union Square Dental,” “we,” “us,” or “our”) respects your privacy. This Website Privacy Policy explains how we collect, use, disclose, and protect information through https://www.unionsquaredental.com and related webpages and forms that we operate (collectively, the “Site”). It also describes choices and rights that may be available to you.
- Scope and Relationship to Our HIPAA Notice
This policy applies to information collected through the Site. It does not necessarily apply to independent websites or services linked from the Site, or to information collected wholly offline.
When information we maintain qualifies as protected health information (“PHI”), our use and disclosure of that information is governed by HIPAA, other applicable health-privacy laws, and our Notice of Privacy Practices. This Website Privacy Policy is not a Notice of Privacy Practices and does not replace it. If this policy conflicts with the Notice of Privacy Practices with respect to PHI, the Notice of Privacy Practices controls.

Information you provide
Depending on how you use the Site, we may collect:
- Contact information, such as your name, email address, telephone number, and mailing address;
- Appointment information, such as date of birth, whether you are a new or existing patient, requested service or reason for the appointment, referral source, and messages;
- Virtual-consultation information, including descriptions of your concerns and photographs or other files you choose to upload;
- Communications you send to us and information you provide when calling or emailing the Practice;
- Information submitted in patient-registration or payment workflows, subject to the notices of the service operating that workflow; and
- Any other information you voluntarily submit through the Site.
Information collected automatically
When you visit the Site, we and service providers may automatically collect information such as your Internet Protocol (“IP”) address, browser type, device type, operating system, general location inferred from IP address, referring page, pages viewed, links clicked, timestamps, and interactions with Site features. Cookies, tags, local-storage technologies, server logs, and similar tools may be used for these purposes.
Information from other sources
We may receive information from vendors that help us operate forms, scheduling, patient registration, billing, security, website analytics, or communications, and from other persons when they are authorized to provide information on your behalf.

We may use information to:
- Respond to questions, appointment requests, referral communications, and virtual-consultation submissions;
- Communicate about scheduling, services, office operations, forms, payments, and patient care;
- Provide, coordinate, document, and improve dental and administrative services;
- Operate, maintain, troubleshoot, secure, and improve the Site;
- Understand general Site usage and performance;
- Detect and prevent spam, fraud, abuse, security incidents, and unlawful activity;
- Comply with legal, regulatory, professional, recordkeeping, and reporting obligations; and
- Establish, exercise, or defend legal claims and protect the rights, safety, and property of patients, visitors, the Practice, and others.
We do not use identifiable dental photographs or PHI in public advertising or testimonials without any separate authorization required by applicable law.
- Cookies, Analytics, and Other Website Technologies
The Site currently uses Google Analytics to help us understand general Site traffic and usage. Google may receive device and usage information, including IP address and page interactions, when this service loads. The Site also uses Google reCAPTCHA to help prevent spam and abuse; Google Maps to display location information; Google-hosted fonts; and UserWay accessibility technology. Pages may include embedded videos or links to social-media platforms. These providers may collect information through their technologies under their own privacy notices.
You can often control cookies through browser settings, although disabling certain technologies may affect Site functionality. You may also use Google's browser add-on for opting out of Google Analytics. Browser “Do Not Track” signals are not standardized, and the Site may not respond to them. Where a legally recognized opt-out preference signal applies to our activities, we will process it as required by law.

- Within the Practice to dentists, staff members, and contractors who need the information for authorized purposes;
- To service providers that support website hosting, forms, email, scheduling, patient registration, billing, payments, security, accessibility, analytics, records, and other operational functions;
- To other healthcare providers, laboratories, insurers, benefit plans, and persons involved in payment or care, as permitted or required by applicable law;
- To professional advisers such as attorneys, accountants, auditors, consultants, and insurers;
- To government agencies, regulators, courts, law enforcement, or other persons when we believe disclosure is required or permitted by law or needed to protect rights, safety, or security;
- In connection with a merger, sale, financing, reorganization, or transfer of all or part of the Practice, subject to applicable healthcare and privacy requirements; and
- With your direction, authorization, or consent.
- Linked and Independent Services
The Site links to independent services, including a patient-form service and a payment service. Information you provide directly on an independent service is governed by that service's privacy notice and terms, as well as any applicable agreements with Union Square Dental. The Site may also link to social media and other external websites. We encourage you to review their privacy notices before providing information.
- Data Retention
We retain information for as long as reasonably necessary for the purposes described in this policy, to maintain appropriate dental and business records, to comply with legal and professional obligations, to resolve disputes, and to enforce agreements. Retention periods vary based on the type of information, whether it becomes part of a patient record, applicable law, and operational needs.
- Data Security
We use administrative, technical, and physical safeguards designed to protect information in light of its nature and the risks involved. No website, transmission method, or storage system is completely secure, however, and we cannot guarantee absolute security. Use the Site only through a device and connection you trust. Do not use website forms for emergencies or submit information the form does not request.
- Children and Information Submitted by Parents or Guardians
The Site is intended for a general audience and is not directed to children under 13. A parent or legal guardian may provide information concerning a minor when seeking dental services for that minor. We do not knowingly collect personal information directly from a child under 13 through the Site without authorization from a parent or guardian. If you believe a child submitted information without appropriate authorization, contact us.
- Your Choices and Privacy Rights
You may contact us to request access to, correction of, or deletion of personal information collected through the Site. We will consider and respond to requests as required by applicable law. Certain information may be retained or handled under HIPAA, dental-record laws, professional obligations, or other legal exceptions. If the California Consumer Privacy Act, as amended by the California Privacy Rights Act (“CCPA”), applies to Union Square Dental and to the information at issue, California residents may have rights to know or access personal information, correct inaccurate information, delete certain information, receive information about collection and disclosure practices, opt out of sale or sharing, limit certain uses of sensitive personal information, and receive nondiscriminatory treatment for exercising privacy rights. Some information, entities, or activities may be exempt from the CCPA, including certain medical information governed by HIPAA or California medical-privacy law.
To submit a request, contact us using the information in Section 13. We may need to verify your identity and authority before completing a request. You may use an authorized agent where permitted by law, but we may require proof of authorization and identity verification. We will not discriminate against you for exercising a right provided by applicable privacy law.
- California “Shine the Light”
California Civil Code section 1798.83 permits certain California residents to request information regarding disclosure of personal information to third parties for their own direct-marketing purposes. Union Square Dental does not disclose personal information collected through the Site to third parties for their own direct-marketing purposes without authorization.
- Changes to This Policy
We may update this policy from time to time. We will post the revised version on the Site and update the effective date. If required by law, we will provide additional notice or obtain consent before applying a materially different practice to previously collected information.
